In consideration for myself and for the members of my family including, any minor children, or dependent person in my care, being permitted to participate in the activities of Camp Maplewood Inc., Maplewood Country Day Camp, and/or Maplewood Enrichment Center, their/its affiliates, officers, director, employees and assigns, all hereinafter described as “Maplewood”, I agree to the following Waiver and Release: I acknowledge that running, jumping, climbing, boating, pedaling (riding a pedal kart), and all activities associated therewith have inherent risks, hazards and dangers for anyone and that the risk of injury can be significant. I willingly agree to comply with the stated and customary terms, rules and conditions for participation and take responsibility for their obligations to comply with the stated and customary terms, rules and conditions for participation. I further agree to observe and obey all posted rules and warnings and to follow any oral instructions or directions given by Maplewood, its employees, representative and agents

As I am attending Maplewood facilities and participating in Maplewood activities, on behalf of myself, my heirs, issue and executors, I hereby knowingly and intentionally waive and release, indemnify and hold harmless, Maplewood, their directors, officers, agents, employees and volunteers from and against any and all claims, actions, causes of action, liabilities, suits, costs or expenses (including attorneys fees) and freely assume all risks both known and unknown, even if arising from the negligence of other participants or any other third parties present at the premises.

In the event of an injury to myself or any members of my family, including any minor children during the described activities, I give permission to Maplewood or to the employees, representatives, or agents of Maplewood to arrange for any necessary medical treatment/attention for which I shall be financially responsible for.

All claims arising from participating in the above activities shall be interpreted and construed under the laws of the Commonwealth of Massachusetts.

The invalidity or unenforceability of any provision herein, whether standing alone or as applied to a particular occurrence or circumstance, shall not affect the validity or enforceability of any other provisions.

I have carefully read, clearly understand, and voluntarily sign this waiver and release.
